Pulmonary Artery Denervation for Pulmonary Artery Hypertension.
نویسنده
چکیده
W hile the seminal advance in the treatment of pulmonary artery hypertension (PAH) over the past 2 decades has been pharmacological targeting of dysfunctional endothelium-derived pathways that contribute to the characteristic vascular remodeling of this condition irrespective of etiology (1), several novel interventional techniques have also been useful in selected patients. Atrial septostomy, which decompresses the pressureand volume-overloaded right heart and increases left heart filling and systemic perfusion, albeit at the expense of arterial oxygen desaturation (2), has been used in PAH patients with severe right heart failure refractory to diuretics and in regions where lung transplantation is not an option. More recently, balloon angioplasty has shown promise as a treatment for both inoperable Chronic Thromboembolic Pulmonary Hypertension (CTEPH) and for patients in whom the operative risk is deemed to be unacceptably high (3). In this issue of JACC: Cardiovascular Interventions, Zhou et al. (4) provide evidence of a pivotal role of the sympathetic nervous system in the pathogenesis of PAH and demonstrate that pulmonary artery denervation (PADN) produces both hemodynamic and structural improvement in an animal model of PAH.

Pulmonary Artery Denervation Reduces Pulmonary Artery Pressure and Induces Histological Changes in an Acute Porcine Model of Pulmonary Hypertension
BACKGROUND Pulmonary arterial hypertension is a devastating disease with high morbidity and mortality and limited treatment options. Recent studies have shown that pulmonary artery denervation improves pulmonary hemodynamics in an experimental model and in an early clinical trial. We aimed to evaluate the nerve distribution around the pulmonary artery, to determine the effect of radiofrequency ...
متن کاملBenefits from the correction of vitamin D deficiency in patients with pulmonary hypertension
Background: Vitamin D (Vit D) is linked to various conditions including musculoskeletal, metabolic and cardiopulmonary diseases. However, it is not clear whether correction of vit D deficiency exerts any beneficial effect in patients with pulmonary hypertension. Methods: This study was a prospective uncontrolled longitudinal study. Patients with pulmonary hypertension and vit D d...

While pulmonary thromboembolism is common, thromboembolic pulmonary hypertension is very rare. The present case is a 35 year old woman with chronic thromboembolic pulmonary hypertension, who presented with severe dyspnea and leg edema, following an earlier thrombotic event of 10 years earlier, after her her second childbearing. She also had a history of the first childbearing complicated with c...
متن کاملStatins and Pulmonary Hypertension in Chronic Obstructive Lung Disease
Chronic obstructive lung disease (COPD) is a chronic multisystem disease with a considerable burden. One of its most common complications is pulmonary artery hypertension (PAH). It has been demonstrated that the development of PAH is correlated with decreased quality of life and survival. Different medications have been proposed for the treatment of PAH, among which one can name statins. Howeve...
متن کاملEffect of lung denervation on pulmonary hypertension and edema.
Under conditions of high pulmonary artery pressure and flow produced in dogs by a controlled systemic artery shunt, pulmonary edema occurred after denervation of the lung at arterial pressures that were tolerated without edema before denervation. Left atrial pressures were not altered by the shunt, which involved only one segment of lung. Observation of animals with shunts for \],i to 10 months...

Background and purpose: Pulmonary hypertension (PH) has been reported in hemodialysis patients, but there are scant studies about its prevalence and mechanisms. The aim of this study was to determine the prevalence of pulmonary hypertension in hemodialysis patients and to study some of its possible etiologic factors. Materials and methods: Ïn this cross-sectional study, the prevalence of p...
